School Meals and Household Eligibility Form
South Jefferson Central School District participates in the Community Eligibility Provision (CEP) or Provision 2 in a non-base year. This means all students receive school meals and milk at no cost, regardless of household income or whether this form is completed. However, completing this form may help your family qualify for additional state and federal benefits, including the Summer Electronic Benefits Transfer (EBT) program. Please read the instructions on the back of the form and complete only one form per household. Sign the form and return it to the school listed above. If you have questions or need assistance completing the form, please contact South Jefferson CSD at 315-232-4519.
