School-Based Health Center Enrollment is Going Digital!

We're excited to announce that enrollment for the North Country Family Health Center School-Based Health Centers is moving to a digital process, making it easier and more convenient for families to enroll and complete annual paperwork.

If your child is NEW to the School-Based Health Center: (NEVER BEEN ENROLLED) 
Please complete the short questionnaire. Once submitted, we'll text you a secure link to complete your child's enrollment forms.

If your child is ALREADY ENROLLED:
You will receive a text message with a secure link to complete your child's annual enrollment forms. No additional action is needed until you receive that message.

Paper enrollment forms will continue to be available upon request and on our website for families who prefer a paper option.
